4/5/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Client upon arrival to the home setting. Upon arrival to the home setting, Client appeared to be in a good mood. QMHP-C engaged Ms. Shabazz in a discussion to determine any new behaviors since the last session. Ms. Shabazz reported that Client feels as though he his is younger brother parent as he likes to hit on him. QMHP-C informed mother that his behaviors would be addressed during the session. 2. Behavior: Client participated in the session as he was angry with his mother for her telling on him at the beginning of the session. After redirection and communication of his behaviors and how they are not acceptable within the home, he seemed to get happy as QMHP-C provided him with a piece of candy. 1.Intervention: QMHP-C provided Client with information on anger and other emotional ties that are connected with things that he is experiencing due to all that he is undergoing. QMHP-C conducted an activity and asked Client to provide QMHP-C to express his feelings in an assertive manner. QMHP-C processed with Client his feelings and processed each with Client once he explained to QMHP-C the reason why he was feeling the way he is. 2. Intervention: QMHP-C challenged Client to list several positive things he can say and think about when he is feeling sad, angry or depressed to his grandmother or an authority figure. QMHP-C role-played with Client to support him with identifying what "triggers" are and how to cope when upset. QMHP-C educated Client that by him expressing these feelings will in hopes to assist him with releasing some of the negative thoughts and energy he is experiencing. 1.Response: Client appeared to be receptive to QMHP-C’s interventions and services that was provided to him during the session. Through completing the activity, Client was able to verbalize his feelings and thoughts as well as identify what face he displayed today. Client indicated to QMHP-C that he does not think that he is his younger brother parent but he makes him upset because he does not let him play with things in the home when they are to be sharing. 2.Response: Client mentioned to QMHP-C that he just get angry all the time because people are always being mean to him. During the role-playing activity, Client was able to express to QMHP-C coping skills to assist when he is angry and upset.
